The Yale Health Plan (YHP) serves students with a wide array of medical services, including primary and specialty care services, emergency care, and mental health and counseling services.  More information about the Yale Health Plan can be found on their website athttp://www.yale.edu/uhs.

Medical Records.  A medical history form will be among the forms you receive this summer. You must have this form completed and submit it order to begin your classes at Yale.

Insurance: All full-time Yale students are required to have adequate hospitalization and specialty care medical coverage. You will be automatically enrolled in and charged a fee for the Yale Health Plan Hospitaliza-tion/Specialty Coverage.  However, you are permitted to waive the plan by completing a waiver form and showing evidence of comparable alternate coverage. The annual single student premium for the YHP Hospitalization/ Specialty Coverage and Prescription Plus Coverage Plan was $840 for the 1999-2000.

Immunizations Certain immunizations are required of all Yale students in order to register for classes., including measles, rubella and mumps. The requirements are listed on  http://www.yale.edu/uhs or  http://www.yale.edu/uhs/forms/index.html

Dental Care.  Dental insurance is not available so you will be responsible for the full cost of any dental work you have done while in New Haven.  If you know you need dental work it is advisable to have that taken care of before you leave home.

Family/Spouse Health Care.  It is extremely important that any member of your family who accompanies you to the U.S has adequate health insurance and access to health care. The Yale Health Plan will provide coverage for a student’s family for an additional charge.  The 1999-2000 charge for a two person family was $3000 and $4344 for family of 3 or more..  In preparing your budget, you must make certain that you can meet this expense of health insurance.

If you have children who will be attending public schools, you will be required to show proof of immunization, including the three D.P.T (diptheria, pertussis, tetanus).   Be sure to bring copies of your medical records and those of accompanying family members.

Finally it is advisable to carry traveler’s insurance while in route to the U.S. if your home insurance will not cover the expenses of emergency medical care.
